What is Home Health?
Imagine if healthcare professionals including doctors, nurses, physiotherapists, etc, could provide medical care or other healthcare services such as wound care, assistance with medication, rehabilitation and physical therapy, etc, to you or your loved ones right in the comfort of your home. That is what home healthcare is.
It is the delivery of health services as directed by a doctor, with such delivery done in a safe, professional, and cost effective manner, in the comfort of the client’s home. These services in the home may become necessary due to acute illness, accidents, long term health conditions, permanent disabilities, terminal illness or the impact of aging.
Often, patients are referred to the home healthcare company or agency for follow-up care after discharge from the hospital or for management of a chronic or terminal illness. The home healthcare personnel will visit and assess the patient’s healthcare needs; and working with the patient’s doctor, draw a care plan that would best benefit the patient. The Home care giver will provide the services as contained in the care plan, and update the doctor on the patient’s status as often and as timely as needed
